Hepburn is the only one to have done it, period, and she is still the only one with four. Ingrid Bergman has three Oscars, but one of those was for Supporting Actress (Murder on the Orient Express).

Streep only has two, and one of those is a Supporting one. She often laughs in interviews that people constantly comment that she has the most nominations in history (13) but never point out that she has lost the Oscar the most times.

This one is harder: Who is the first actor to win three Oscars, regardless of lead or supporting?

Nope, he only has two.

Walter Brennan won three Supporting trophies back in the 30's and 30's.

Besides him, Bergman, and Hepburn, Jack Nicholson is the only other actor with three.
